Have a apt with a Endocrinologist to bring down my A1C .

They send me a 8 page questionnaire , have to put name , address , SS# [etc]. on every page .

The kicker is two of the questions were , Do you have a gun in the house, and is it safe from children?

I wanted to write down that i have one on me as i fill this out .

My best guess is that it affects your Copay. You have a gun , your copay is cheaper .

This is happening way too often to folks on our gun forums. It appears this was one of those hidden little secrets with the Obamacare quagmire. Those questionnaires cover all medical problems and if you claim mental illness and confess to owning guns, expect a visit. (at least this is what I've been told)

WTF?
I admit that I've never had forms that asked about whether or not I have guns. And it is completely unrelated to medical stuff so it is none of their business!

Editing to add that where I live most people have guns-- hunting is pretty big around here-- so I think they generally assume that people will have guns.

Hunting is big here to Z. You get those tests yet ?

They took blood and a cervical scraping (which goes without saying was not fun at all) but no results yet. I go back on the 8th for the ultrasound to see if I have any abnormal growths.

I'm still surprised that a question about guns was on a medical form though. Just ludicrous.
